5 months of age he had bilateral medial rectus recessions augmented formula. He is seen here postoperative at 2 years of age wearing full hypermetropic correction. He is orthotropic and the Bruckner red reflex is symmetrical. (E) Author’s son at 17 years of age with straight eyes. He has high grade stereo acuity. He wore full hypermetropic correction until age 11 years when he grew out of the hypermetropic correction. Note this author treated his own son, including performing the strabismus surgery (less expensive that way).
Sensory esotropia can be caused by an infantile cataract, retinoblastoma, optic nerve hypoplasia, or any other cause of infantile visual loss. 00 sphere or more, give spectacles with the full correction (see Infantile Accommodative Esotropia later in this Chapter). If not significantly hypermetropic, observe until the infant is 6 to 9 months old for spontaneous resolution. Some patients will show an increasing esotropia and they should be considered for early surgery if the deviation becomes constant and is ≥40 PD seen on at least two consecutive exams (see congenital esotropia below).
Clinical Evaluation Patients with poor vision do not fixate well, therefore, prism cover testing will not be accurate. If ductions are full you can estimate the deviation by placing the appropriate base out prism over the “good” eye to move the amblyopic eye out to a cosmetically acceptable primary position (Hering’s law). Estimate the deviation for distance by placing a base out prism over the good eye to move the deviated eye to primary position. If ductions are limited, use the Krimsky test for near with the prism over the deviated eye.
